Nearly 1.3 million people in the United States will be diagnosed with cancer this year.   

 

 

One in two men and one in three women, or approximately 120 million Americans, will develop cancer over their lifetime.    

 

 

One of every four deaths in America is caused by cancer. It is the second leading cause of death in the U.S., exceeded only by heart disease.    

 

 

Since 1990, approximately 15 million new cancer cases have been diagnosed.    

 

 

More than 1,500 people die of cancer per day - about one person every 56 seconds.   

 

 

Nearly 5 million lives have been lost to cancer since 1990.    

 

 

The National Institutes of Health estimates overall costs for cancer in 2000 at $180.2 billion.     

 

 

Our government currently invests only $3.2 billion to find cures, treatments and prevention strategies for cancer. Of these funds,   only 2% are invested in research and development. This amounts to $12.03 per year per person.    

 

 

8,900,000 people are alive today as a result of progress in cancer research.    

 

 

Over the past 30 years, the five year cancer survival rate has risen from 38% to 60% according to the National Coalition for Cancer Research.

Here are three OPTIONS  to get  the Team Summary Results data back to Hoop Champs, Inc. in order for us
to process all the data updating the databases, fill the orders for the incentives for you and your players,
and to present a check on behalf of all the participating teams to:

OPTION 1:  (Standard Mail)
Collect all the Player Pledge Forms and  complete the Team Summary Results Form which you received in the mail upon enrolling in the Fundraiser.  Send all the forms in, along with a check for 33% of the Amount raised, as indicated on your Team Summary Results Form, made payable to:  "GAMES WON - LIVES SAVED" and mail everything  to:

OPTION 2: (Microsoft Excel)
If you are comfortable with Microsoft Excel Spreadsheets, you can download a "template" ready for you to update and then when completed, you can save it to your PC (remembering the file name which you saved it) and send your Team Summary Results Form in as an attachment to an email. 

NOTE:  THE SPREADSHEET IS CLEAN OF ANY VIRUSES AND IS SAFE TO OPEN AND USE !!!

Once you open the spreadsheet - enable the macros - you only need to fill in the cells which ARE NOT shaded in color and then save it to your PC.    Calculations will be made for the 67% Team / 33% Fundraiser split...

Upon completion of the form you would then send an email to:
coachjay@hoopchamps.com and attach the Excel Spreadsheet in the email.  Upon receiving it, I'll email you back notifying you that the file was received with all the appropriate data fields updated.

OPTION 3:  (ON-LINE)
If you would prefer to transfer the data to the Team Summary Results From that  I've created through this web-site, you can do so by entering the data from the copy of your form you have already completed.
